How to Report a Claim to Nationwide Agribusiness

You may report a claim to Nationwide Agribusiness 24 hours a day, 7 days a week by using one of the following methods:

By Email: newclaim@nationwide.com
NEW! Email new commercial losses to: newclaim@nationwide.com. Attach a copy of the loss report or First Report of Injury (for workers comp) to the email and hit send.

PLEASE NOTE: If there is ANY URGENCY to the claim that would require immediate attention, please call the Claims Customer Service Center at (800)228-6700, Option 2 (see By Phone below).

By Phone:
 » For all claims, call (800)228-6700, Option 2.
 » For Workers Compensation claims, fax the required state-specific First Report of Injury     form to (800)842-1482. Please fax within 24 hours of the injury.
 » Report a new Farmowners Claim by calling (800)421-3535. You will be asked to provide     voice prompts. Simply follow the prompts by saying “Claims” and then “Farmowners” as     requested by the prompts.
 » Or, call your agent.

By Fax:
 » Fax your new commercial claims loss form to (800)842-1482.
 » For Workers Compensation claims, fax the required state-specific First Report of Injury     form to (800)842-1482. Please fax within 24 hours of the injury.
 » Please fax medical bills and supporting documents to (800)842-1482.

By Mail:
